The forms can be obtained by the following options:
Downloading the application form from the website and pay by Demand Draft a sum of Rs. 1150/- while submitting the form.
The forms can be obtained by hand from BLSIMS Campus, Bahadurgarh, Haryana on payment of Rs. 1100/- through Cash.
